Reconstructive Surgery & Complex Wounds

When a case is labeled "too complicated", patients come to Dr. Topaz. Decades of experience in reconstruction and healing of difficult wounds, with technologies he himself invented.

Restore & Repair

Reconstructive surgery

Reconstructive surgery restores not only form and function, but confidence and quality of life. Dr. Topaz headed a hospital plastic surgery unit for 26 years, with thousands of reconstructive operations behind him.

Breast Reconstruction

Breast reconstruction after mastectomy, with implants or autologous tissue, in coordination with the oncology team. Unique global knowledge of implant safety as founder of the International Breast Implant Registry.

Skin Tumors: Excision & Reconstruction

Excision of benign and malignant skin tumors with immediate reconstruction, including delicate repairs of the face, nose and ears using skin grafts and local flaps.

Scars & Defects

Surgical revision of painful, restricting or prominent scars, treatment of congenital and acquired defects and rehabilitation after injuries and accidents.

A Global Center of Expertise

Complex & hard-to-heal wounds

Dr. Topaz is considered an international authority on the closure of complex wounds. He lectures and operates in this field worldwide, and the technologies he invented serve physicians in many countries. Patients arrive at the clinic after months of suffering from non-healing wounds, and find an answer.

Chronic & Hard-to-Heal Wounds

Diabetic wounds, pressure ulcers, venous ulcers and post-surgical wounds that fail to close, including patients discharged home with dressings only. The approach combines gradual closure with the TopClosure® system, regulated negative pressure therapy and oxygen enrichment, and when needed a portable vacuum system that continues the treatment at home. The goal: early closure, without skin grafts or major operations, keeping amputation risk away.

Infections After Cardiac & Chest Surgery

Treatment of sternal infections following open heart surgery: one of the most complex fields in medicine, for which Dr. Topaz developed a unique protocol combining local delivery of ultra-high concentration antibiotics.

Salvage of Infected Pacemakers & Defibrillators

A first-of-its-kind conservative approach to preserving infected cardiac implantable devices, sparing elderly and complex patients a dangerous replacement operation. Published in the professional literature and applied successfully in dozens of cases.

Abdominal Wall Infections & Defect Repair

Management of infections of the abdominal cavity wall and primary repair of large abdominal wall defects after extensive resection for infection or malignancy, using gradual tension-relief closure techniques.

Limb Soft Tissue Injuries & Limb Salvage

Closure of extensive soft tissue injuries of the limbs after trauma and accidents, preserving limbs that were candidates for amputation through staged closure and advanced wound care.

Trauma & Combat Wounds

Rich experience in closing extensive trauma wounds, road accident injuries and combat wounds, including the development of methods designed for mass-casualty scenarios and emergency medicine.

The full range of wounds Dr. Topaz treats

  • Pressure ulcers of all stages, including deep tissue injury
  • Deep sternal wound infection after cardiac surgery
  • Infected pacemakers and implanted cardiac devices
  • Venous ulcers and chronic leg wounds
  • Dehisced surgical wounds that reopened after an operation
  • Open abdomen and abdominal wall defects
  • Major wound defects after resection of large tumors
  • Traumatic, orthopedic and road-accident wounds
  • Combat and blast injuries
  • Hand surgery wounds
  • Wounds of vascular surgery patients
  • Deep and partial-thickness small-to-medium burns
  • Support of skin grafts and flaps
  • Hard-to-heal wounds in elderly, complex patients
  • Complex wounds in children, together with pediatric teams
Is your family member hospitalized with a wound that will not heal? Dr. Topaz can be invited for a bedside consultation at the hospital, to evaluate the wound and build a closure plan together with the treating team.
Good to know: some complex wound treatments are provided within public hospitals. At the consultation, Dr. Topaz will direct you to the appropriate treatment track, private or public.

The Technology Behind the Success

When the surgeon is also the inventor

The TopClosure® system invented by Dr. Topaz enables primary closure of large, high-tension wounds that once required skin grafts and multiple operations. The system harnesses the viscoelastic properties of the skin for gradual, controlled stretching.

Alongside it, Dr. Topaz developed the RO-NPT method: negative pressure therapy enriched with oxygen and continuous irrigation, accelerating the healing of infected wounds. The combination is applied today in hospitals in Israel, China and other countries.

  • Primary closure of wounds once considered "unclosable"
  • Reduced need for skin grafts and flaps
  • Shorter hospitalization and recovery
