Infected Cardiac Devices (CIED)
Localized infection of a pacemaker or defibrillator pocket has traditionally required complete device and lead extraction: a major procedure carrying substantial risk in elderly or medically complex patients. Dr. Topaz developed CITA-MIS (also referred to in the literature as the Topaz Procedure), a minimally invasive alternative combining limited debridement of the infected pocket, controlled local delivery of ultra-high-concentration antibiotics (CITA) with systemic therapeutic drug monitoring, wound sealing under regulated negative pressure (RNPT), and TopClosure®-secured closure, preserving the implanted system in carefully selected patients. The approach was evaluated in a peer-reviewed study in the Journal of the American College of Cardiology (2023) as a salvage strategy for selected cases, not a universal substitute for extraction.
Pressure Ulcers (Deep Tissue Pressure Injuries)
Stage 3-4 deep tissue pressure injuries are chronic, nonsterile wounds, often harboring polymicrobial, biofilm-associated infection and osteomyelitis, that conventionally require wide excision and flap reconstruction, with prolonged off-loading and frequent dehiscence and recurrence. Dr. Topaz's integrated protocol reverses the conventional sequence: after conservative, tissue-preserving debridement, the wound is closed primarily by TopClosure® tension-relieved approximation, without waiting for sterility, and residual infection is then treated inside the closed compartment by CITA, delivering culture-directed antibiotics at ultra-high local concentrations under daily therapeutic drug monitoring, within a sealed ROINPT environment that suppresses anaerobic growth through supplemental oxygenation, provides continuous irrigation-assisted drainage, and insulates the wound from external and fecal contamination. Approximation of the wound margins also collapses dead space, concentrating the delivered antibiotic into a smaller compartment. The approach is flap-sparing, avoids fecal diversion, and shifts care from a surgery-centered to a rehabilitation-centered pathway.
Diabetic Wounds and Infected Amputations
In diabetic foot wounds and infected or dehisced amputation sites, the priority is arresting infection while preserving limb length and function. Dr. Topaz applies the combined TopClosure–ROINPT–CITA strategy: mechanical closure, regulated negative-pressure wound management, and targeted local antibiotic delivery, as an adjunct to standard vascular and endocrinologic diabetic foot care, in carefully selected patients.
Open Abdomen
Open abdomen management after trauma, sepsis, or major abdominal surgery requires staged closure that balances infection control against the risk of abdominal compartment syndrome. Dr. Topaz uses TopClosure®-based progressive tension-relief closure, integrated where indicated with ROINPT, to achieve gradual, controlled abdominal wall approximation and reduce dependence on complex mesh or flap reconstruction.

Infected Sternal Wounds
Deep sternal wound infection after cardiac surgery carries substantial morbidity. Dr. Topaz's protocol, developed and refined over decades of dedicated clinical experience with this specific complication, combines CITA (targeted local delivery of ultra-high-concentration antibiotics into the infected mediastinal and sternal compartment) with ROINPT and TopClosure®-assisted closure.
Complex Trauma and Combat Wounds
Large soft-tissue defects from trauma, combat injury, or oncologic resection often default to skin grafting or flap reconstruction. TopClosure® uses the viscoelastic properties of adjacent skin to achieve immediate or gradual primary closure of large, high-tension wounds, reducing dependence on grafts, flaps, and tissue expanders in appropriately selected cases, including mass-casualty and combat settings.

Surgeon, scientist, inventor and educator
Dr. Topaz is a graduate of the Ben-Gurion University Faculty of Medicine, holds a Master's degree Cum Laude from Tel-Aviv University and a Ph.D. in science. He trained in plastic surgery at the Chaim Sheba Medical Center (Tel Hashomer) and completed a fellowship in plastic and reconstructive surgery in the United States.
For 26 years he headed the Plastic Surgery Unit at the Hillel Yaffe Medical Center. Today he operates as a senior volunteer surgeon at Israel's leading hospitals, including Ichilov, Kaplan, Meir and Schneider, alongside his private clinic in Ra'anana and surgical work at Herzliya Medical Center and Assuta.

Dr. Topaz is a recipient of China's Friendship Award (2009), the highest honor the People's Republic of China bestows on foreign experts, for his contribution to advancing wound care medicine.
Dr. Topaz volunteered to treat casualties of the devastating earthquake in Sichuan Province, and received recognition from the Premier of China. On his initiative, a first-of-its-kind sister-hospital agreement was signed between Israel and China.
